It is looking for the 'lgsl' table in a 'modified_serverq' database.

You either need to import the included .SQL file into your nuke database to create the table.

Or if you used a separate database before, then you need to edit the lgsl_config.php and put in the db.

If you point LGSL to a database where LGSL 5.0 was installed, keep in mind the structure has changed, so you need to re-create the table.
